9774 Private William Charles MIDDELDYK, a 22 year old Dentist from 22 Bent Street, Paddington, Sydney, New South Wales. He enlisted in the AIF on 10 January 1916 and was allotted to the Divisional Train Reinforcements 16 which embarked from Sydney, New South Wales, on board HMAT SS Makarini on 5 April 1916. During the Great War MIDDELDYK Returned to Australia, 1 February 1918.
